Four Corners Health Department Workshops


Beat the Pack (Tobacco Cessation); 4 sessions, 1 hour each

This program is offered in a group setting.  Participants will learn about the different options for quitting and encouraged to set a quite date before the class ends.  The final sessions are intended to help participants to stay quit.

Diabetes Prevention Program (year-long program)

This is a one-year program in a class/group setting.  Participants must qualify for the class by lab results or by taking a diabetes indicator quiz.  This program teaches skills and provides tools needed to make a change for life.  In this program, participants will learn strategies to incorporate healthy eating and moderate physical activity into daily life, as well as techniques to handle stress and strategies to overcome barriers to a healthy lifestyle.  By the end of the program, most participants reach 5%-7% weight loss and have increased activity to 150 minutes per week.

This program can be offered in-person, or through the Health and Lifestyle Training (HALT) online self-paced format.

Freedom From Smoking (Tobacco Cessation); 8 sessions, 2 hours each

This program, from the American Lung Association, is offered in a group setting.  It provides a step-by-step process to quit for good.  During each session, activities and assignments will provide skills and techniques to support quitting.  All exercises are intended to help gain control over behavior, working through the quit process individually and in a group.

The first three sessions will help participants prepare to quit.  Session 4 will be the Quit Day.  Sessions 5 through 8 are to help participants stay quit.

The program is more extensive and a higher fee, but it is ranked as the most effective smoking cessation program.  Often called the “Gold Standard” of tobacco quitting programs.

Living Well With Chronic Disease; 6 sessions (once per week), 1.5-2.5 hours each

This interactive workshop is facilitated by trained leaders and helps people, or caregivers of people, with one or more lifelong health condition to take steps to manage their health and live happier, healthier lives.  It is an evidence-based, self-management class that provides tools, skills, and support for participants to make lasting lifestyle and behavioral changes.  Topics include action planning, communication, healthy eating, physical activity, medication management, and MORE!  Disease self-management teaches individuals strategies for identifying challenges and solving problems associated with their illness(es).

Living Well With Chronic Pain; 6 sessions (once per week), 1.5-2.5 hours each

This interactive workshop is facilitated by trained leaders and helps people, or caregivers of people, with chronic pain to take steps to manage their health and live happier, healthier lives.  It is an evidence-based, self-management class that provides tools, skills, and support for participants to make lasting lifestyle and behavioral changes.  Topics include action planning, communication, healthy eating, physical activity, medication management, and MORE!  Disease self-management teaches individuals strategies for identifying challenges and solving problems associated with chronic pain.

Living Well With Chronic Diabetes; 6 sessions (once per week), 1.5-2.5 hours each

Narcan/Naloxone Training 1 session, 30 minutes

We offer a 30-minute training (including time for questions) that includes PowerPoint, hand-outs, posters, and a sample Naloxone/Narcan policy and plan for your organization.

Question, Persuade, Refer (QPR) Suicide Prevention Training; 1 session, 1.5 hours

Just as people trained in CPR and the Heimlich Maneuver help save thousands of lives each year, people trained in QPR learn to recognize the warning signs of a suicide crisis and how to question, persuade, and refer someone to help.  Say “Yes” to saving the life of a friend, colleague, sibling or neighbor.

Stepping On; 7 sessions (once per week), 2 hours each

Stepping On is a 7-week program designed for people who are living at home and have experienced a fall or are concerned about falling.  This is an evidence-based program that is effective to reduce falls by up to 31%.  The program incorporates strategies to implement positive lifestyle changes to keep you independent, upright, and active.  This program also helps to keep individuals independent and in their own homes.

Tobacco Quitting Information Session; 1 session, 30 minutes to 1 hour

This session is for those who want to learn what is available locally and statewide to help tobacco users quit.  Different options are reviewed, including the state Quitline, American Cancer Society, medical provider support and medications, and classes (Beat the Pack, Freedom From Smoking, and other area options).  Participants learn about an online Readiness Quiz they can use to guide their decision in picking a quit date.

Wellness Recovery Action Plan (WRAP); 1 full day session, or 2 half day sessions

A Wellness Recovery Action Plan (WRAP) is a wellness tool anyone can use to get well, stay well, and make their life more the way they want it to be.  Create a personalized system of wellness and an action plan to care for yourself and others!
