Polk County Health Department can provide Homemaker Chore Services such as:
Light Housekeeping (dusting, vacuuming, dishes, laundry, sweeping, mopping), Personal Care (meal preparation, bath assistance, medication set-up, errands/shopping, medical escort), and Companion/Respite services.
What does the admission process look like?
*They will come to your home
*Assess your individual needs
*Determine a plan to meet your individual needs
*Schedule services at acceptable times to you and your homechore worker
Fees for homemaker chore services are covered by private pay.
Private insurance and Medicare WILL NOT cover this service.
Scheduling for services needs to be done at least a week in advance.
Services can only be provided in Polk County.
Services will be provided by a Certified Nursing Assistant and/or Medication Aide.

Taking care of yourself - is one of the best ways to help your loved ones!
Respite is help for the helpers. Respite is a break for unpaid family caregivers caring for a loved one with special needs. Respite allows the family caregiver time away to care for their personal needs. Research shows that when family caregivers take time for themselves, they improve their physical and emotional health.
You can use respsite to: Go to the grocery store, visit the doctor, have lunch with a friend, take a nap, or anything else you need or want to reduce stress.
A family caregiver is a person who provides ongoing care for an individual unable to care for themself due to age, physical or mental disability, or illness. If your loved one cannot be left alone due to a special need, we can help.
Caregiving is rewarding, but also demanding and stressful. We offer information, education, and support. We are the place to contact to discuss your respite needs.
Who provides respite? There is some flexibility in finding providers. Your local respite coordinator can assist you with finding a provider in your area. You may be able to use family members, friends, or neighbors as paid providers. Depending on funding source, other possibilities include: organizations, camps, a trusted agency, a local volunteer-led organization or group, volunteer-led school-based program, equine program, faith-based or other approved activities. While your loved one is attending an activity, you are getting a break-and that's what respite is all about!

The Polk County Health Department has been in existence since 1979 The services offered are available to all residents of Polk County, regardless of income status. Funding comes from a number of sources including: private contracts, private payment based on a fee for services, donations, grants, and tax support from the Polk County Government.
Services of the department include: Homemaker/Chore Services, School Nursing, Health Screenings, Health Education Program, Pedicure Clinics, Blood Pressure Clinics, Public Flu Vaccines, Immunization Clinics - Adult and Childhood, Drug/Alcohol Testing, Lifeline, Tai Chi: Moving for Better Balance.

York General Home Health offers a broad range of services that support clients in their own home. It can serve as an alternative to hospitalization or nursing home placement. Medicare coverage is available to individuals certified by their physicians as home bound and in need of skilled services.
Medicare definition of home-bound:
The individual can leave his/her home only for short periods of time and only for medical appointments, special occasions, or religious services.
Great effort is required for the individual to leave the home.
The individual needs assistance to walk with a cane, walker, or wheelchair.
Individuals are not considered homebound if they can effortlessly walk or drive a vehicle.
