COVID-19 Update: Take Home Tests Available

Four Corners Health Department has received a number of take home COVID tests to be available throughout the District. The test kits are free to the public and come with a free option to receive a documented lab result if desired.

Tests can be picked up at:

  • Four Corners Health Department at 2101 N Lincoln Ave in York
  • Polk County Health Department at 330 N State Street in Osceola
  • Seward Memorial Library at 233 S 5th Street in Seward
  • Hruska Memorial Library at 399 N 5th Street in David City

If a documented lab result is needed, before opening the test kit, a person can go online to There a Certified eMed Guide will watch and help you take the test. If this is done, the test will be considered verified and can be used for official purposes. If the person chooses not to use the virtual option, the test results are for personal use only.

If you have symptoms and need a test kit, the drive thru window at the Seward library is available to get a kit. Alternatively, in York call 402-363-2621 and staff will meet you in the Four Corners Health Department parking lot with a test(s). People can receive up to 6 test kits at a time.

COVID vaccine clinics are held each Friday at Four Corners on 2101 N. Lincoln Avenue in York from 12PM to 4PM. All three vaccines and boosters (Pfizer, Moderna, and Johnson & Johnson) are offered at each clinic, including Pfizer vaccine for children ages 5 and over. There is no charge for the vaccine.  Registration at  is appreciated. Remember to bring your vaccine card.
